Guided Dental Plan (HMO)
- Lower monthly rate
- Negotiated fees for services received from dentists in the DHMO network.
- Participants must choose a primary provider and use plan dentists and specialists to receive benefits.
- Available only in select areas of AL, AZ, AR, CA, CO, CT, DE, FL, GA, IL, IN, IA, KS, KY, LA, MA, MD, MI, MN, MO, NE, NJ, NY, NC, OH, OK, OR, PA, SC, TN, TX, UT, VA, WA and WI. Benefits vary for California residents in the dental HMO plan, view the benefits.
